Birth & Death Registration (English)

Subject: Notification of required documents and fees for Registration of Births and Deaths.

Ref: (a) The Ministry of Foreign Affairs letter No-19.00.0000.530.57.013.23.181 dated: 15 March 2023/(b) Local Government Department’s letter No- 46.04.0000.101.22.001.21-278 dated: 22 February 2023/This is for the information of all expatriate Bangladeshis staying in Turkey and concurrent accredited countries, the following Birth and Death Registration fees and documents will be required for Registration of Birth and Death:

List of documents required for Birth Registration:

A). The following documents should be attached with the application submitted within 45 (forty-five) days of the person's birth:

(i) Proof of Place and Date of Birth, clearance certificate from the concerned medical institution or attested copy of birth certificate obtained from the said institution or birth attendant's certificate or any other certificate: And

(ii) Birth Registration certificate or National Identity card or passport or any other proof of the person's permanent address; or

Up-to-date copy of proof of payment of tax in domicile declared as permanent address of the person being registered or parents or Grand parents; And

Birth Registration Certificate or National Identity card of the registered person's parents.

B).  In case of submission of application submitted after 45 (forty-five) days of the person's birth should be accompanied by the following in addition to the above-mentioned documents:

(i) Duly filled in application form attested by EPI personal or a copy of EPI card, if applying within 05 (five) years of birth;
(ii) Certificate issued by a competent doctor to proof date of birth if applied after 5 (five) years of birth; or
(iii) Educational Certificate of Primary Education Completion, Junior School Certificate and Secondary School Certificate or similar examination conducted by the Education Boards of Bangladesh.
Documents required for Death Registration:
Application submitted within 45 (forty-five) days of the person's death should be accompanied by one or more of the following certifications or documents;
(a) Death Certificate issued by the concerned Medical Institution or Physician; or
(b) Certificate for concerned health worker; or
(c) Attested copy of the post-mortem report of the deceased; or
(d) A copy of the burial or cremation receipt issued by the concerned caretaker of the graveyard or tomb or cremation ground; or
(e) Death Certificate issued by the concerned Union Parishad member or Councilor of the Municipality or City Corporation or Cantonment Executive Officer in case of Cantonment Board; or
(f) A Certificate issued by the Imam performing Janaza or the priest of the funeral of the deceased in case of other religions;
(g) If the documents referred to in (a) to (f) cannot be obtained, copies of such other documents relating to the death as the Registrar deems appropriate;
(h) Birth Registration Certificate of the deceased; And
(j) Copy of Birth Registration certificate or National Identity card of the person applying for death registration.
 *** The application submits after 45 (forty-five) days of the person's death should submit a write-up explaining the reason for delay.
